Over the past 20 years, there has been an increasing interest in surgery for the anterior cruciate ligament (ACL) deficient knee. With this increase, there has been the concomitant development of surgical technique, in particular, the minimally invasive arthroscopically-assisted procedure. As an alternative, we have developed a technique that does not require the use of the increasingly expensive equipment for an arthroscopically-assisted reconstruction. The Mayday 'rhino horn' jig is a hand-held device that can accurately and simply place the tibial tunnel required for cruciate reconstruction. Use of the jig reduces soft tissue dissection and allows for a faster operative procedure without the need for a thigh tourniquet. These factors have allowed us to offer ACL reconstruction as a day case procedure.
