Early electrocardiographic diagnosis of acute myocardial infarction in the presence of ventricular paced rhythm

To identify electrocardiographic findings associated with evolving acute myocardial infarction (AMI) during ventricular pacing, we compared the prerandomization electrocardiograms of 17 patients with ventricular pacing and confirmed AMI from the Global Utilization of Streptokinase and t-PA for Occluded Coronary Arteries-I trial with those of 17 controls with paced rhythms and stable coronary artery disease. The presence of ST-segment elevation greater than or equal to 5 mm in leads with negative QRS polarity had high specificity (88%) and fair sensitivity (66%) for the diagnosis of AMI (positive likelihood ratio; p = 0.025).
