Combined laparoscopic and endoscopic resection techniques have been applied for benign and early malignant gastrointestinal tumors. Endogastric surgery is a variant of these techniques which combines endocavitary laparoscopic resection with flexible endoscopic guidance. By placing trocars directly into the stomach, a number of procedures seem to be possible such as endogastric resections in anatomic regions not suitable for so-called combined endoscopic laparoscopic transgastric resections. In order to illustrate this technique, the excision of a gastrointestinal stroma tumor located near the esophagogastric junction by percutaneous endogastric resection is reported and discussed. Although the indications for such procedures are limited, the endogastric access is a promising technique that reflects the continuous evolution of open cavitary to totally noninvasive intracavitary surgery.
