This article presents the authors' current approaches to the management of patients harboring carcinoma of the pancreas, with emphasis on clinical and surgical interpretations. For those areas of diagnosis and therapy that are controversial, the authors endeavor to provide support and rationale for their biases. Comments are limited to ductal carcinoma of the pancreas because of the differences in the natural history and management of nonductal pancreatic and periampullary tumors of the pancreas.
