The evolution of care for the chronically critically ill patient has resulted in the development of specialized ventilator units in long-term acute care hospitals. By developing a multidisciplinary approach to patient care, these units have helped optimize outcomes for this challenging patient population. This article reviews how these units evolved over the last 10 to 15 years, the system of payment and regulation, and the future of these units in the managed care environment.
