One of the problems in the treatment of rectal cancer surgery has been the inability of surgeons to optimize local tumor control. The basic conventional procedure, involving blunt dissection, has often resulted in incomplete removal of mesorectal tissue, with high local recurrence rates. Recently, local control and survival have been extensively improved by the introduction of the total mesorectal excision (TME) technique first described by Heald. In this article we discuss the role of total mesorectal excision in the management of rectal cancer and review developments in the treatment of mobile rectal cancer, the introduction of TME surgery, and quality assurance of surgery. We also discuss the Dutch trials in which TME surgery has been performed.
