摘要
Discharge from an acute core hospital stresses continuity, an important aspect of care for frail, older persons. Secondary analysis of qualitative data from two studies indicated that gaps in communication were a fundamental source of discontinuity. This article describes gaps in discharge planning, identifies the constraints encountered in planning for discharge, and discusses possible solutions.
