Controlling the sugar bowl - Regulation of glucose homeostasis in children

摘要
The control of glucose metabolism is a dynamic process and highly regulated. This article proposes a model that depicts the regulatory processes under fed and fasting circumstances and applies the model to several common clinical conditions. Glucose enters the glucose pool-the "sugar bowl"-by three potential "faucets," representing glucose rates of appearance from the gut, liver, and kidney. Glucose exits the glucose pool by insulin-dependent and insulin-independent routes (rates of disappearance). Additionally, interactive factors are explored, including hormones, enzymes, substrates, and transporters that maintain the blood glucose concentration within very narrow ranges, in the fed and fasting states. The purpose of this model is to elucidate the dynamic processes involved in glucose homeostasis for the evaluation, diagnosis, and management of children with carbohydrate metabolism disorders.
