Ictal SPECT in temporal and extratemporal epilepsy

Single photon emission computed tomography (SPECT) with HMPAO shows distinctive and rapidly changing blood flow patterns at the time of seizures. We analyzed the pattern of hyperperfusion in the early phase of epileptic seizures in 42 consecutive patients with medically intractable epilepsy. Seizures were induced by bemegride injection. Two ''blinded'' observers identified the location of hyperperfusion. The perfusion changes in ictal SPECT provide consistent and reliable information about medial-lateral differentiation as well as laterality of the seizure. Accordingly, SPECT with HMPAO injection during seizures is a selective method for identifying seizure foci noninvasively.
