The "no-reflow" phenomenon results from microcirculatory spasm and has significant hemodynamic and prognostic consequences. Adenosine is an effective microvessel vasodilator. We describe two cases of successful treatment of no-reflow with intracoronary adenosine after failure of intracoronary calcium channel blockers.
