Health care transition in congenital heart disease: the providers' view point

摘要：

Successful transition of adolescents and adults with congenital heart disease requires collaboration and planning between the pediatric health care team and the adult congenital heart disease health care team. Good communication and an atmosphere of mutual respect are essential in order to create a health care delivery system that maximizes the lifelong potential and function of adults with congenital heart disease. This article reviews various health care delivery models, provides an overview of published recommendations regarding the organization of health care for adults with congenital heart disease, and discusses implications for practice.
