Transtympanic therapy is becoming an important treatment modality for a number of otologic complaints. This article reviews the rationale and history for transtympanic therapy in the treatment of tinnitus. Results in two groups of tinnitus patients are then examined: those with Meniere's disease and those with tinnitus secondary to sudden or noise-induced sensorineural hearing loss. Finally, the difficulties with the current therapies and the questions that need to be addressed both in basic and clinical research are discussed, and where clinicians hope to be in the future with this therapy is examined.
