Our understanding of the pathophysiology of acute pancreatitis has evolved dramatically in recent years. It is now apparent that enzyme-induced, macrophage-derived, extrapancreatic production of cytokines mediates distant organ injury. Future therapies will likely be aimed at 3 targets: prevention of further local pancreatic parenchymal injury, attenuation of macrophage-derived cytokine production, and inhibition of neutrophil-mediated cytotoxicity.
