Since Fowler in 1948 and Schuknecht in 1956 first used intratympanic aminoglycosides for the treatment of vertigo, intratympanic application has also been used for steroids, local anesthetics, oto-protective drugs and other antibiotics. As a result, there has been growing interest in the biology of round window drug absorption and the pharmacokinetics and pharmacodynamics of intratympanic therapy. This article outlines the current understanding of how drugs traverse the round window, how they distribute within the fluids of the inner ear, and how they exert their effect on their target tissues.
