Meconium aspiration syndrome is a common problem in neonates with mortality and morbidity. This article reviews mechanisms of injury and controversies in obstetrical and neonatal treatment, including the use of amnioinfusion, ventilator strategies, surfactant lavage and replacement, and treatment with inhaled nitric oxide.
