重症脑梗死患者行机械通气后呼吸机相关肺炎感染特点及危险因素分析

摘要
目的探讨重症脑梗死患者实施机械通气后发生呼吸机相关性肺炎病原学特点及发生肺炎的危险因素, 为临床诊治提供参考。方法选择2010年6月至2014年6月512例重症脑梗死患者, 114例患者实施机械通气发生呼吸机相关性肺炎, 分析发生呼吸机相关性肺炎患者病原菌分布及耐药性, 并对比两组患者临床资料, 分析肺炎发生的危险因素。结果 114例患者共分离出128株病原菌, 革兰阴性杆菌98株, 占76.56%;革兰阳性杆菌30株, 占23.44%。肺炎克雷伯菌、金黄色葡萄球菌为主要病原菌类型。革兰阴性杆菌对亚胺培南耐药率很低, 革兰阳性杆菌对万古霉素、利福平、复方新诺明耐药率较低。机械通气时间(95%CI=2.124~9.586, P=0.001)、侵入性操作(95%CI=1.823~6.334, P=0.002)、误吸(95%CI=1.221~3.541, P=0.027)、肺部基础疾病(95%CI=1.676~5.297, P=0.013)、营养不良(95%CI=2.497~11.103, P=0.000)是发生呼吸机相关性肺炎的危险因素。结论金黄色葡萄球菌、肺炎克雷伯菌是呼吸机相关性肺炎的主要病原菌, 其耐药机制复杂, 机械通气时间、侵入性操作、误吸、肺部基础疾病、营养不良是发生呼吸机相关性肺炎的危险因素。
