The relationships between insomnia comorbid with medical and psychiatric illness are complex and remain poorly understood. The relationship varies according to the specific comorbidity, as do the relevant treatment options and their effects. Comorbid insomnia has a pervasive role in the course of general medical and psychiatric illness. This article reviews the literature on the reciprocal interactions between insomnia and medical and psychiatric illness and on pharmacotherapy approaches for comorbid insomnia. The focus is on several of the comorbidities that are most common or relevant for the sleep clinician, including major depressive disorder, chronic pain, obstructive sleep apnea, chronic obstructive pulmonary disease, end-stage renal failure, dementia, alcohol use disorders, and comorbid insomnia during terminal illness.
