The care of individuals with diabetes is increasingly being devolved to the primary care and community setting, and being delivered by multiprofessional teams. There is a strong emphasis on the education and training of patients coupled with information technology to assist in the collection and presentation of relevant data. Biochemical parameters play an important role in both the day-to-day management of diabetes and in this process of disease monitoring and review. We have reviewed the recent literature on diabetes management in increasingly devolved settings using primarily HbA1c results as a surrogate measure of clinical outcome. The studies reviewed cover a wide range of study design and the intervention used, which makes a detailed comparison difficult to perform. However, it is clear that patient awareness of the latest HbA1c result in itself improves outcome measures, and that education and training on managing the disease delivered in a devolved setting can improve outcome measures. Point-of-care testing for HbA1c can also contribute to these disease management programs.
