A sero-epidemiological survey for Q fever was conducted among 388 subjects residing in rural area in the North of the Charente, France. These 226 males and 162 females (mean age ± SD = 40.6 ± 16.3 years) were distributed in 3 groups. Group I (55 subjects) included 18 families of stock-breeder, owner of animals with a Q fever positive serology, group II : 33 individuals working on animal products (abattoirs, manufactures, of woolens and animal guts) and group III : 300 control subjects. All sera were tested both by complement fixation (CF) and fluorescent assay (FA) using a phase II antigen (CF : Behring ; FA : bioMérieux). FC assay was less sensitive but more specific than FA. We found a seroprevalence of 8.7 % (34/388). The proportion of seropositivities was significantly higher (p < 0.0001) in group I (32.7 %) and II (30.3 %) than in group III (2.0 %). Age and sex had no influence on the distribution of specific IgG and IgM. This study showed that Q fever was endemic in the North of the Charente area. The importance of the infection was confirmed among a rural population, mainly farmers and stock-breeders. © 1990.
