摘要：

The optimal provision of vascular access for the ESRD patient involves the following: early identification of such patients so that either a primary a/v fistula or a PTFE graft may be created; prospective monitoring of VDP and urea recirculation to detect venous stenoses; intervention with either angioplasty or surgical revision in cases found to have venous stenosis; and development of a quality assurance plan that monitors thrombosis and fistula replacement rates and seeks causes of fistula thrombosis in patients without an anatomic basis for the clotting.
