TISSUE PENETRATION OF TRIMETHOPRIM AND SULFONAMIDES

Trimethoprim is widely distributed in the body, with some intracellular sequestration, which results in an apparent volume of distribution greater than that of the body water. Sulphonamides, on the other hand, are more confined to the blood and extracellular fluid. In most body fluids and secretions the concentration ratio of trimethoprim/sulphonamide is also usually less than in the blood, typically 1:2 to 1:5, although there are wide individual variations. Since sulphonamides alone are not active at low concentrations and a ratio with a low sulphonamide content is unsatisfactory for synergy against the majority of bacteria, even in antagonist-free media, the exact role of sulphonamides in therapy with cotrimoxazole is still open to debate. Trimethoprim zeigt eine gute Gewebsverteilung mit vereinzelter Sequestration, welche zu einem offensichtlich gröβeren Verteilungvolumen als bei alleiniger Betrachtung des Plasmawassers führt. Sulfonamide andererseits beschränken sich mehr auf das Plasmawasser und die extrazellulare Flussigkeit. In den meisten Korperfliissigkeiten und Sekreten sind die Konzentrationen von Trimethoprim/Sulfonamid gewohnlich geringer als im Blut. Typisch sind die Verhaltnisse 1:2 bis 1:5, obwohl es groBere Abweichun-gungen gibt. Da die Sulfonamide allein in geringerer Konzentration nicht wirksam sind und ein Wirkstoffverhaltnis mit niedriger Sulfonamidkomponente in bezug auf den Synergismus unwirksam gegen die Mehrzahl der Bakterien ist, sogar bei antagonistenfreiem Medium, muB die exakte Rolle der Sulfonamide bei Anwendung mit Trimethoprim noch nachgewiesen werden. © 1979 Oxford University Press.
