Open wedge osteotomy in the active sports patient population is widely accepted and leads to good mid-term and long-term results. In contrast to closed wedge osteotomy, open wedge osteotomy has a low complication rate especially with respect to foot drop. Open wedge osteotomy has a significant influence on changing tibia slope and the patella tibial distance. Stable fixation is essential for osseous healing in the corrected position. Additional bone grafting using spacer plates leads to early osseous consolidation and decreases risk of secondary malalignment. Intraoperative alignment measurement using an image intensifier is essential and navigation leads to even more precise correction.
