Twenty-one monocularly aphakic patients, including three from a previous study, were given phakic-aphakic (P-A) spectacles with the proper correction for each eye. Fourteen of these patients made a total environmental adaptation, five a partial adaptation, and two made none. Of seven patients who compared P-A spectacles with the use of a balance lens, three preferred the P-A glasses, two preferred the balance lens, and two found either approach acceptable. Four patients preferred the experimental spectacles to a monocular contact lens. Matching the patient's own phoria, rather than base-in prism power, appears to be the essential requirement for success. That prismatic diplopia as a result of wearing phakic-aphakic spectacles is an insuperable obstacle to ambient stereopsis appears to be disproved; that such spectacles produce an unfusible degree of aniseikonia also seems to be disproved. Further studies aim at equalizing image size as well as at predicting the degree of success with phakic-aphakic spectacles. © 1969.
