Unilateral endometriosis of the ureter is rare and only 11 reports of bilateral involvement have been found in the literature. One case of bilateral endometriosis with acute bilateral ureteral obstruction is reported, as well as one case of unilateral endometriosis that became clinically manifest only 9 years after hysterectomy for endometriosis of the uterus. In both cases, the endometriosis of the ureter could not be detected by diagnostic ureteroscopy. The disease was only diagnosed during open surgical partial ureterectomy and end-to-end anastomosis. After additional therapy with danazol for 6 months, the obstruction of the upper urinary tract in the three localisations had disappeared completely. The specific diagnostic problems, the necessity for an early diagnosis and the indications for hormonal and surgical therapy of endometriosis of the ureter are discussed
