Megestrol acetate is a synthetic progestin that is licensed in the United States for palliation of advanced metastatic endometrial or breast cancer. It is an appetite stimulant in patients with cancer and has been used in a small, open-label study to treat patients with the acquired immunodeficiency syndrome (AIDS) and cachexia. The drug has been considered to be relatively nontoxic. Hyperglycemia has been observed rarely. We report a case of diabetes mellitus induced by megestrol acetate in a patient with AIDS and cachexia. The diabetic state was characterized by relative insulin resistance, necessitating the institution of insulin therapy and the withdrawal of megestrol acetate for glucose control. Rechallenge with megestrol acetate after stabilization of the diabetic state resulted in recurrence.
