IN EVALUATING THE SPINE, magnetic resonance (MR) imaging markedly improves detection and characterization of a wide range of pathologic conditions. Along with technical advances aimed primarily at reducing motion artifact and imaging time, the advent of paramagnetic contrast agents has further strengthened the diagnostic capabilities of MR imaging. Contrast material-enhanced MR imaging has emerged as the primary method for diagnosing many extrinsic and intrinsic lesions of the spine. In this article, we briefly review the enhancement patterns of normal spinal structures. We then discuss application of contrast-enhanced MR imaging to the three major compartments of the spine: extradural, intradural extramedullary, and intramedullary.
