Family histories of 191 patients with systemic lupus erythematosus were obtained to test the hypothesis that the female preponderance among clinical cases of this disease may reflect excessive male fetal wastage among potentially susceptible siblings. Among 581 siblings there was a significant decrease in the sex ratio, the ratio of liveborn male siblings to total liveborn siblings, of those siblings born immediately preceding and subsequent to the index case, a finding which supports the hypothesis.
