溃疡性结肠炎患者肠道菌群多样性及炎症细胞因子变化的研究

目的: 本研究旨在通过对UC患者肠道菌群种类和数量的研究及部分细胞因子活性的研究,探索UC发病机制以期对UC为代表的肠道内源性感染性疾病的发病病因探索和诊断治疗奠定基础。 方法: 1.实验分组和病例选择 实验分为健康组、UC组。 UC组20例,符合溃疡性结肠炎诊断标准,年龄14岁~67岁。溃疡性结肠炎病例诊断标准:表现为持续或反复发作的腹泻,黏液脓血便伴腹痛、里急后重和不同程度的全身症状,病程在4-6周以上,辅助结肠镜检查。 健康组20例,年龄22岁~65岁。经临床上体检证实身体健康,未见有肠道疾病,3个月内未服用过任何抗生素药物。 2.肠道菌群分析方法检测肠道菌群种类与数量。 3.采用ElISA法检测人的外周血IL-6、TNF-α、 IL-10、IL-4细胞因子含量。 结果: 1溃疡性结肠炎患者组菌群与健康对照组相比检测呈现菌群多样性减少,其中,肠球菌,链球菌和消化球菌检测不出; 2溃疡性结肠炎患者组菌群与健康对照组相比数量变化明显,其中,双歧杆菌,乳酸杆菌和韦荣球菌数量明显下降;(P<0.05); 3肠杆菌,类杆菌和梭杆菌数量上升明显(P<0.05);葡萄球菌和酵母菌数量变化不明显。 4溃疡性结肠炎患者组外周血中IL-6含量为307.16±39.21ng/L与健康对照组相比,显著升高。(P<0.01) 5溃疡性结肠炎患者组外周血中TNF-α含量为710.14±30.34ng/L与健康对照组相比,显著升高。(P<0.01) 6溃疡性结肠炎患者组外周血中IL-10含量为4.21±1.23ng/L与健康对照组相比,显著降低。(P<0.01) 7溃疡性结肠炎患者组外周血中IL-4含量为7.32±2.51ng/L与健康对照组相比,显著降低。(P<0.01) 结论: 1溃疡性结肠炎患者肠道菌群多样性减少,数量以革兰阴性菌上升,正常菌群结构被破坏。 2溃疡性结肠炎患者体内促炎细胞因子水平升高,抑炎细胞因子水平下降,免疫平衡被打破。
