心肺运动试验在心力衰竭患者中的应用

摘要
心肺运动试验(Cardiopulmonary Exercise Testing, CPET)是在单纯运动试验的基础上加入了对运动过程中机体气体交换的分析,在肺动脉高压、心力衰竭、运动康复治疗、心脏移植等心血管领域有广泛的应用价值。与单纯运动试验和六分钟步行试验相比,结果更加准确、客观且重复性强。在心力衰竭领域,CPET是心力衰竭患者预后评估的独立预测因子,同时也是心脏移植候选者筛选指南中患者筛选的唯一量化指标。近年来,CPET在肥厚型心肌病、儿童心力衰竭、妊娠合并心脏疾病以及心脏移植术后生存评估方面的价值研究取得了一些新的进展,本文着重在这些方面进行综述。首先,我们探索N末端B型钠尿肽原(NT-proBNP)、超声心动图参数与CPET检查参数峰值氧耗量(Peak VO2)、二氧化碳斜率(VE/VCO2 slope)之间的相关性,本研究共入选了123例心力衰竭患者,原发病因主要为心肌病和冠心病,均在经过积极的抗心衰治疗后,心功能恢复至NYHA≥Ⅱ级时完善CPET检查。研究发现心力衰竭患者的血清NT-proBNP与CPET主要参数Peak VO2(r=-0.33, P<0.001)和VE/VCO2 slope (r=0.49, P<0.001)之间存在显著相关性。NT-proBNP在节点1176pg/ml处,估测Peak VO2<14ml/kg/min的特异性和敏感性分别为76%和64%,阳性预测值为79%,阴性预测值为58%;在节点1817pg/ml处,估测Peak VO2< 10ml/kg/min的特异性和敏感性达到71%和80%,阳性预测值为28%,阴性预测值为96%。提示经过积极的抗心衰治疗后,NT-proBNP仍持续超过1200pg/ml的部分患者心肺运动功能可能已经出现严重损害,应及时完善CPET检查,以免贻误治疗时机。但由于NT-proBNP在预测Peak VO2和VE/VCO2 slope中重度减低(Peak VO2<14或10ml/kg/min, VE/VCO2slope>34或40)方面变异度较大,且阳性预测值不能令人满意,故NT-proBNP在判断心力衰竭严重程度方面不能替代CPET检查。其次,对于合并左心疾病相关肺动脉高压的心力衰竭患者,本研究进一步对CPET参数VE/VC02slope与超声心动图估测肺动脉收缩压(Systolic Pulmonary Artery Pressure, sPAP)之间的相关性进行研究,发现VE/VCO2slope>34.2提示患者sPAP>50mmHg的可能性,特别对于运动耐量尚存(Peak VO2>14ml/kg/min)的患者,其特异性和敏感性分别达到93.8%和71.4%。提示VE/VCO2slope明显升高的患者应及时进行有创血流动力学评估sPAP,以免贻误心脏移植最佳时机。
