表没食子儿茶素没食子酸酯减轻大鼠局灶性脑缺血再灌注损伤的机制

摘要
【目的】建立大鼠局灶性缺血再灌注损伤模型,并探讨表没食子儿茶素没食子酸酯(EGCG)对大鼠局灶性缺血再灌注损伤的保护作用及其机制。 【方法】采用线栓法阻断大鼠右侧大脑中动脉2小时后拔除进行再灌注,制备大鼠局灶性缺血再灌注损伤模型。雄性健康SD大鼠54只,体重200~250克,随机分为假手术组(sham组)、缺血再灌注损伤组(I/R组)及EGCG组(E组)。大鼠脑缺血开始后向右侧侧脑室注射0.1mg的EGCG,sham组及I/R组给予等量生理盐水。再灌注后24小时进行神经功能障碍评分,2,3,5-三苯基氯化四氮唑(TTC)染色法测定脑梗死体积,ELISA法测定脑组织细胞因子TNF-α、IL-1β,分光光度计测定脑组织MPO活性、SOD活性,硫代巴比妥酸法测定脑组织MDA含量,HE染色观察脑组织病理形态变化,原位末端脱氧核苷酸转移酶标记法(TUNEL)测定神经元凋亡,免疫组化法测定Bcl-2、Bax。 【结果】缺血再灌注损伤24小时后,大鼠神经功能缺损体征明显,细胞因子TNF-α、IL-1β增高,凋亡细胞增多。与I/R组比较,EGCG组神经功能缺损改善,脑梗死体积减小,TNF-α、IL-1β水平降低,MPO活性降低、SOD活性升高及MDA含量减少,组织形态学损伤减轻,凋亡细胞减少,组织Bcl-2蛋白表达增多,Bax减少。 【讨论】大量研究证实,氧化应激、炎症反应及细胞凋亡等与脑缺血再灌注损伤的发生和发展有关,也有很多文献报道通过减轻氧化应激和炎症反应、减少细胞凋亡等而起到脑保护作用。本研究发现脑缺血再灌注损伤经EGCG处理后炎症反应和氧化应激减轻、细胞凋亡减少,所以考虑EGCG改善神经功能缺损和病理特征及减低梗死体积等的脑保护作用可能与上述机制有关。 【结论】EGCG可以减低大鼠局灶性缺血再灌注后的炎症因子TNF-α、IL-1β水平和减轻氧化应激,从而能减轻缺血再灌注所致的脑组织损伤起到脑保护作用。
