踝关节CPM对脑卒中患者下肢运动功能康复的疗效观察

摘要
目的观察踝关节持续被动运动(CPM)改善脑卒中患者下肢运动功能的效果,并探讨其临床意义。方法本研究纳入60例脑卒中患者,随机分为对照组和治疗组。对照组给予常规康复治疗,包括运动疗法、电动起立床治疗、低频电疗法、针灸治疗和踝足矫形器治疗等;治疗组在给予常规康复治疗的基础上,增加踝关节CPM治疗。两组患者均采用“医院-家庭-医院”的康复治疗模式,康复流程一个周期为医院治疗3周,家庭康复治疗1周,医院治疗期间除患者的康复治疗外,同时对其家属进行康复治疗技术的培训,确保其掌握基本的康复治疗技术,出院后进行家庭康复,总共循环进行3个周期,共3个月。分别于治疗前和治疗3个月后对患者行下肢肌痉挛评估、下肢Fugl-Meyer运动功能评定(FMA-L)和10m步行时间评估,然后对结果进行统计学分析,观察患者下肢运动功能的改善情况。结果经过3个月的治疗后,两组患者下肢肌痉挛评分较治疗前降低(P<0.05)且治疗组优于对照组;两组患者FMA-L较治疗前提高(P<0.05),髋、膝、踝关节的运动评分也均较治疗前有所提高(P<0.05),且除髋关节外,治疗组均优于对照组(P<0.05);两组患者10m步行时间均较治疗前缩短(P<0.05),且治疗组优于对照组(P<0.05)。结论1、踝关节CPM通过缓慢、持续的牵伸痉挛的肌肉,可以有效缓解脑卒中患者下肢的痉挛;2、踝关节CPM可以增加脑卒中患者患侧踝关节的主动关节活动度,促进分离运动的出现,改善下肢运动功能。3、踝关节CPM能够改善患侧下肢本体感觉,加大患足与地面的接触面积,提高患者的站立平衡功能,促进患者步行能力的恢复。
