:
外界刺激对机体的效应是环境与机体交互作用的结果。机体在接受外界环境刺激后,会启动相应的应答反应来维持内环境稳态,使机体免受损伤。在接触重金属化学物如汞、铅、镉、锌等后,机体会迅速启动一类金属特异的应答反应(metal response),其特征在于合成一类小分子量环境应激蛋白以保护机体免受外源性化学物的损伤。这一类应激蛋白即金属硫蛋白(metallothionein,MT)。 镉(Cadmium,Cd)是一类有毒的金属,可引起多器官损伤,长期低剂量接触下,肾功能损伤是镉毒性的靶效应(critical effect),预防镉靶效应的出现对于预防其它器官的毒效应具有重要的意义。大量的研究表明MT与镉在体内的运输、代谢和解毒等密切相关,但是许多方面还没有阐明。本研究选择我国环境和职业镉接触人群为研究对象,通过流行病学研究方法,分别从基因、蛋白和抗体水平来观察MT、MT抗体与镉致肾功能损伤的关系。 为了更好地预防镉诱导的肾功能损伤,有必要寻找反映镉接触和肾功能损伤的早期的生物标志物,并据此估算出现早期损伤的接触水平来制定相应的接触限值。本研究以职业镉接触人群为研究对象,分别以血镉(BCd)、尿镉(UCd)和总镉摄入量(TTCd)为镉接触标志物;尿β2-微球蛋白(UB2M)、尿N-乙酰-β-D-氨基葡萄糖苷酶(UNAG)为肾小管损伤效应标志物,尿白蛋白(UALB)为肾小球损伤效应标志物,观察了尿金属硫蛋白(UMT)与上述接触和效应标志物的关系,并通过基准剂量(Benchmark dose,BMD)的方法计算出上述指标的BMD和BMDL(基准剂量下限值)。结果发现UMT与镉接触标志物BCd、UCd、TTCd间有很好的一致性。肾功能损伤人群的UMT水平和高UMT尿异常率明显高于肾功能正常人群。随着尿镉水平增加,上述尿蛋白、尿酶浓度和异常率相应增加,表现出明显的剂量-效(反)应关系。职业镉接触人群UMT的尿镉BMDL(3.1μg/g Cr)高于UNAG(2.7μg/g Cr),但低于UB2M(3.4μg/g Cr)和UALB(4.1μg/g Cr)。提示UMT可作为职业镉接触人群肾功能损伤的早期生物标志物。 相同接触条件下,镉引起的肾损伤效应可以存在个体差异,目前认为机体对毒物反应性的差异可能主要与机体所携带的环境应答基因的多态性有关。作为一种金属应答蛋白,MT的基因多态性与镉肾毒性间呈现何种作用,目前尚不清楚。本研究选择我国华东某环境镉接触人群为研究对象,以BCd、UCd和TTCd作为镉的接触标志物,UNAG及其同功酶UNAGB、UB2M、UALB及尿视黄醛结合蛋白(URBP)为效应标志物,观察了MT1A650A/C(re:11640851)位点单核苷酸多态性(SNP)与镉致肾损伤的关系。发现MT1A-rs11640851位点A/A、A/C、C/C基因型和等位基因分布在对照组和接触组间均无明显差异(P>0.05),上述三种基因型在对照组的分布频率分别为20.6%、56.4%和23.0%。随着尿镉增加,上述肾损伤标志物异常率明显增加,表现出明显的剂量-反应关系。组间或1∶1配对分析未观察到肾功能损伤人群和肾功能正常人群间MT1A-rs11640851基因分布频率具有统计学意义(P>0.05)。但是,本研究发现当尿镉低于5.0μg/g Cr时,C/C+A/C基因型携带者出现肾小管损伤的危险性是A/A基因型携带人群的8.4倍(95%CI:1.67-42.16),而当尿镉高于5.0μg/g Cr后,MT1A-rs11640851不同基因型对镉诱导的肾功能不产生明显的修饰作用(P>0.05)。上述结果提示MT1A-rs11640851位点基因多态对镉的肾损伤效应存在遗传修饰作用,表现为低镉接触水平下,A/C和C/C基因型携带者增加肾小管损伤的易感性。 人们处于生活和生产环境之中,容易受到多种环境因素的刺激,一方面,外源性抗原作用或环境有害因子刺激可诱导机体产生针对自身抗原的自身抗体。另一方面,机体内也在多种天然自身抗体,也可能对环境因素的效应产生修饰作用。有研究发现正常人和一些疾病状态人外周血中含有抗MT的自身抗体。此类金属硫蛋白抗体(metallothionein antibody,MT-Ab)与镉毒效应,尤其是肾功能损伤有何关系,目前尚不清楚。本研究分别选择我国某镉冶炼厂作业工人为职业研究对象,某社区糖尿病人群为环境研究对象,观察机体镉负荷、血浆MT-Ab及肾功能损伤的关系。 职业人群研究发现,镉接触组中UCd(3.16μg/g Cr)、BCd(9.28μg/L)、UB2M(81.17μg/g Cr)、UALB(7.03 mg/g Cr)水平明显高于对照组,随着职业镉接触水平增加,UB2M、UNAG和UALB含量和异常发生率明显增加。对照组与接触组间MT-Ab滴度无明显差异。MT-Ab滴度并不随接触剂量增加而增加,与血镉、尿镉和总镉摄入量间无明显联系(P>0.05)。在接触组中MT-Ab与UNAG和UB2M呈正相关,相关系数分别为0.30和0.22(P<0.05)。相同机体镉负荷下MT-Ab高滴度人群较低滴度人群更易出现肾小管损伤,比值比为4.20(95%CI:1.21-14.54)。上述结果显示,职业性镉接触与肾功能损害间存在剂量反应关系,而与血浆MT-Ab间无明显关联。血浆MT-Ab高水平者更容易出现肾脏损伤,提示血浆MT-Ab可作为职业接触人群镉致肾功能损伤的易感性标志物。 一些疾病状态如糖尿病可以增加机体对毒物的敏感性,人群流行病学研究发现镉可以增加糖尿病患者的肾损伤效应。本研究观察了上海某糖尿病人群机体镉负荷、肾功能损伤及血浆MT-Ab的相互关系。发现肾小管效应标志物UB2M随着UCd和MT-Ab增加而明显增加,在男性和女性人群中UB2M与UCd、MT-Ab均呈正相关(P<0.05),但UCd与MT-Ab间并无明显相关性(P>0.05);UCd、MT-Ab与肾小球效应标志物UALB间并无明显相关性(P>0.05)。Logistic回归分析发现糖尿病人群肾小球损伤的主要影响因素为血糖水平,当毛细血管血糖水平高于6.1mmol/L后,肾小球损伤危险性较低于6.1mmol/L人群增加2.75倍(95%CI:1.12-6.74)。影响肾小管损伤的危险因素主要为吸烟、尿镉和MT-Ab水平。与不吸烟者相比,吸烟者肾小管损伤的危险性增加3.51倍(95%CI:1.14-10.08):当尿镉超1.0μg/g Cr,肾小管损伤的可能性增加3.34倍(95%CI:1.17-9.53);与低滴度MT-Ab相比,高滴度人群出现肾小管损伤的危险性增加5.56倍(95%CI:2.25-13.73)。上述结果提示职业和环境镉接触条件下,血浆MT-Ab可以增加肾小管损伤的易感性。
