老年2型糖尿病患者肌肉减少症的发生及相关影响因素分析

摘要
目的(1)调查老年2型糖尿病(T2DM)患者肌肉减少症(Sarcopenia)的患病情况,比较老年T2DM患者与非2型糖尿病(non-T2DM)者肌肉相关指标和肌肉减少症的患病率是否存在差异。(2)进一步探讨老年T2DM患者肌肉减少症发生的相关影响因素,以促进镇江地区乃至我国老年人群对肌肉减少症的认识,养成健康生活习惯,实现对老年人群尤其是老年T2DM患者肌肉减少症的二级预防和早期干预。方法共纳入研究对象330例,年龄≥60岁,根据1999年WHO有关T2DM的诊断标准分为T2DM组165例和non-T2DM组165例,使用人体成分分析仪(TANITA-MC180)以多频生物电阻抗法(BIA法)检测四肢骨骼肌质量(ASM),以四肢骨骼肌质量指数SMI评估肌肉质量,采用JAMAR握力测功器检测握力评估肌肉力量,4米行走测验检测步行速度评估身体活动能力。根据亚洲老年人肌肉减少症工作组(AWGS)有关肌肉减少症的诊断标准,评估镇江地区老年人群肌肉减少症的发生情况,并比较老年T2DM组与non-T2DM组肌肉质量、肌肉力量、身体活动能力和肌肉减少症患病率的差异。根据AWGS有关肌肉减少症的诊断标准进一步将老年T2DM组分为S arcopenia组24例和non-Sarcopenia组141例,均行75g口服葡萄糖耐量试验(OGT T),检测空腹和餐后2h血糖(FPG、2h PBG)、糖化血红蛋白(HbA1c)、胆固醇(TC)、甘油三酯(TG)等相关生化指标,由专人测定体重(W)、身高(H)、臀围(HC)、腰围(W C)等人体参数,计算体质量指数(BMI)、腰臀比(WHR),并记录T2DM病程。结果(1)老年T2DM组SMI低于non-T2DM组[(7.08±1.20)kg/m2 vs.(7.40±1.15)kg/m2,P<0.05],两组年龄、性别、BMI、握力和步速差异没有统计学意义(P>0.05);老年男性T2DM组SMI低于non-T2DM组[(7.92±1.04)kg/m2 vs.(8.23±0.80)kg/m2,P<0.05],两组年龄、BMI、握力和步速差异无统计学意义(P>0.05);老年女性T2DM组SMI显著低于non-T2DM组[(6.25±0.64)kg/m2 vs.(6.83±1.01)kg/m2,P<0.01],T2DM组HS显著低于non-T2DM组[(22.46±4.15)kg vs.(25.97±6.72)kg/m2,P<0.01],两组年龄、BMI和步速差异无统计学意义(P>0.05)。(2)330名老年人发生肌肉减少症者35人,约占10.61%,T2DM组和non-T2DM组肌肉减少症的患病率差异有统计学意义[14.55%vs.6.67%,χ2=5.40,P<0.05];按性别分组老年男性和女性T2DM组和non-T2DM组肌肉减少症的患病率差异均无统计学意义[17.07%vs.9.33%,12.05%vs.4.44%,P>0.05]。(3)老年T2DM组中Sarcopenia组年龄大于non-Sarcopenia组[(74.71±5.54)岁vs.(69.09±5.80)岁,P<0.05];Sarcopenia组T2DM病程大于non-Sarcopenia组[(14.33±4.97)年vs.(9.35±5.08)年,P<0.05];Sarcopenia组BMI小于non-Sarcopenia组[(21.89±2.57)kg/m2vs.(24.21±3.17)kg/m2,P<0.05];Sarcopenia组HbA1c大于non-Sarcopenia组[(9.21±1.34)%vs.(8.33±1.55)%,P<0.05]。(4)单因素分析显示不同年龄组、病程组、BMI组及HbA1C组肌肉减少症的患病率差异有统计学意义(P<0.05)。多因素非条件Logistic回归结果显示年龄[OR=4.251,95%CI(1.715~10.534),P<0.01]、病程[OR=2.603,95%CI(1.180~5.744),P<0.05]、HbA1C[OR=3.998,95%CI(1.224~13.060),P<0.05]是老年T2DM患者肌肉减少症发生的独立危险因素。结论(1)老年T2DM患者与非糖尿病人群相比,骨骼肌肉质量降低,肌肉减少症的患病率增加;老年男性T2DM患者骨骼肌肉质量较非糖尿病人群降低;老年女性T2DM患者骨骼肌肉质量和力量均较非糖尿病人群显著降低;T2DM可能在肌肉减少症的发病机制中发挥一定作用。(2)年龄、病程、HbA1C是老年T2DM患者肌肉减少症发生的独立危险因素,在临床上针对年龄大、病程长、长期血糖控制不佳的老年T2DM患者应及时进行肌肉减少症的早期筛查以实现早期预防和及时干预。
