四妙勇安汤对糖尿病大鼠肢体缺血的治疗作用及对Akt、p38MAPK通路的影响

摘要
目的 1.研究四妙勇安汤对糖尿病后肢缺血大鼠的治疗作用; 2.研究四妙勇安汤对糖尿病后肢缺血大鼠Akt、p38MAPK蛋白表达水平的影响; 3.探讨糖尿病下肢缺血模型的建立及评估。 方法 1.40只Wister大鼠分为随机四组:对照组、对照治疗组、糖尿病组、糖尿病治疗组,对照组、对照治疗组建立单纯后肢缺血模型;糖尿病组、糖尿病治疗组建立糖尿病后肢缺血模型,对照组、糖尿病组给于溶药蒸馏水灌胃,对照治疗组、糖尿病治疗组给予四妙勇安汤灌胃治疗; 2.分别于术前、术后即刻、术后3、7、14天监测下肢血流变化,术后15天取腓肠肌组织免疫组织化学CD31染色血管内皮细胞观察新生血管密度,及Western Blot法检测内皮细胞Akt、p38MAPK蛋白变化情况。 结果 1.糖尿病组、糖尿病治疗组大鼠腹腔注射链脲佐菌素后3天后空腹血糖明显较对照组、对照治疗组增高,7天后空腹血糖达稳定水平,对照组、对照治疗组、糖尿病组、糖尿病治疗组分别为:腹腔注药后3天:6.24±0.45mmol/L、6.10±0.37mmol/L、24.87±4.47mmol/L、26.09±3.95mmol/L;腹腔注药后7天:6.60±0.87mmol/L、6.17±0.39mmol/L、26.77±3.60mmol/L、28.10±4.15mmol/L(对照组、对照治疗组比糖尿病组、糖尿病治疗组,P<0.05)。后肢缺血模型采用手术方法,缺血效果明显,各组大鼠术后即刻与术前血流值比较差异明显,有统计学意义,但组间差异无统计学意义。 2.糖尿病组血管损伤严重,血管重建能力显著降低,第7天对照组、对照治疗组、糖尿病组血流比值(%)为45±4、49±3、36±3,第14天血流比值(%)为63±6、66±4、45±4,差别均有统计学意义(对照组、对照治疗组比糖尿病组,P<0.05);糖尿病组术后15天血管密度明显减低,Akt蛋白表达明显降低,p38MAPK蛋白表达明显升高,差别均有统计学意义(对照组、对照治疗组比糖尿病组,P<0.05); 3.四妙勇安汤能够明显改善血流比值(%)(第7天糖尿病组、糖尿病治疗组分别为36±3、48±6;第14天糖尿病组、糖尿病治疗组分别为45±4、61±4,P<0.05),并增加毛细血管密度(个)(糖尿病组、糖尿病治疗分别为9.1±2.0、16.4±2.9,P<0.05)及毛细血管密度比值(手术侧/非手术侧)(糖尿病组、糖尿病治疗分别为0.62±0.15、0.86±0.18,P<0.05);四妙勇安汤增加Akt蛋白表达,降低p38MAPK蛋白表达,差别均有统计学意义(糖尿病组与糖尿病治疗组蛋白相对光密度比值:Akt:0.20±0.07、0.28±0.56;p38MAPK:1.18±0.23、0.99±0.04,P<0.05)。 结论 成功建立糖尿病模型,该方法可模拟2型糖尿病的多饮、多尿、及高血糖特征,后肢缺血模型采用手术方法,缺血效果明显,二者结合可作为糖尿病肢体缺血模型;四妙勇安汤能够改善糖尿病下肢缺血血运重建过程,增快血管新生速度,其作用机制可能是通过上调Akt蛋白表达,下调p38MAPK蛋白表达及其分子通路实现的。
