补阳还五汤对大鼠脑缺血组织周围新生血管整合素α5β1表达的影响

摘要
目的探讨脑缺血再灌注后不同时间点整合素α5β1与血管新生的关系,探讨益气活血剂与整合素α5β1的表达及血管新生之间的关系。方法1.制备中药煎剂。将补阳还五汤方药组成根据功效分为全方,益气方,活血方。2.选取健康雄性大鼠,应用线栓法建立大鼠大脑中动脉闭塞(MCAO)模型。建模成功后,根据随机对照原则,将大鼠分为假手术组,模型组、补阳还五汤组,益气组和活血组。再将每组分为4d、7d、14d三个亚组。按照大鼠改良神经功能缺失进行评分。将评分1-3分的大鼠纳入最终实验分组。假手术组和模型组灌服生理盐水,其余各实验组灌服益气活血各方。应用Longa五级评分法评价大鼠神经功能缺损程度;TTC染色测定在脑缺血后4d,7d,14d的脑组织梗死体积的变化;苏木素伊红(HE)染色观察不同时间点脑组织细胞的形态变化;免疫组织化学法测定各实验组缺血区脑组织中CD34和α5β1的表达,分析其阳性细胞的数量,比较微血管密度;Western Bloting检测脑缺血组织中,整合素α5β1的表达。结果1.神经功能评分。补阳还五汤组、益气组和活血组与模型组比较,神经功能评分在4d时无明显差异,在7d,14d时明显降低,差异显著(P<0.05)。其中补阳还五汤组明显低于益气组和活血组(P<0.05)。益气组和活血组差异无统计学意义(P>0.05)。2.梗死体积。4d时各实验组梗死体积无明显差异(P>0.05)。在7d,14d时补阳还五汤组、益气组和活血组与模型组比较,梗死体积明显缩小(P<0.05)。其中补阳还五汤组明显低于益气组和活血组(P<0.05),益气组和活血相比无明显差异(P>0.05)。3.缺血侧微血管密度。4d时各实验组微血管密度无明显差异。在脑缺血后7d,14d益气活血各组与模型组相比,脑缺血周围组织新生血管密度明显升高,与益气组和活血组比较,补阳还五汤组升高明显,差异显著(P<0.05)。4.整合素α5β1表达。大鼠脑缺血后整合素表达明显升高。脑缺血后4d,益气活血各组整合素α5β1表达略高于模型组,但差异无统计学意义(P>0.05);脑缺血后7d,14d益气活血各组与模型组比较,可见整合素α5β1表达明显升高,差异显著(P<0.05);其中补阳还五汤组明显高于益气组和活血组(P<0.05)。结论大鼠脑缺血后,缺血区周围组织新生血管密度增加,整合素α5β1表达明显增强,益气活血治疗可明显改善大鼠神经功能,减小梗死体积,有利于提高大鼠脑组织α5β1的表达,促进血管密度增加,是血管新生的重要通路。
