N-乙酰半胱氨酸对高血压心脏重构的影响

摘要
目的探讨N-乙酰半胱氨酸(NAC)对高血压大鼠心脏重构的影响及其作用机理。 方法结扎SD大鼠腹主动脉造成高血压模型,随机分为四周组及八周组,前者再次随机分为手术组及非手术组,而后者进一步随机分为手术组、非手术组及手术+NAC组(NAC 75mg/d,共4周)。两组大鼠分别于四周及八周时分别测定心脏功能、颈动脉血压、左室质量,观察心脏组织中还原型谷胱甘肽及基质金属蛋白酶(MMP)-2和MMP-9的分布,并测定心脏组织胶原含量的变化。 结果与手术组相比,手术+NAC组大鼠收缩压明显降低[(161±6)vs(198±10)mmHg,P<0.05]、左心室收缩功能明显改善[EF(77.2±6.4)%vs(90.6±2.7)%,SF(56.53±4.37)%vs(41.02±5.84),P均<0.05]、心肌组织中还原型谷胱甘肽的含量显著增加[(84.5±7.1)%vs(38.6±3.4)%P<0.05],而左室质量(1.33±0.05 vs 1.46±0.1g,P<0.05)及心肌组织中的基质金属蛋白酶分布均显著降低[MMP-2(56.9±6.1)%vs(96.8±4.8)%,P<0.05;MMP-9(43.7±8.1)%vs (90.6±6.7)%]、心肌胶原含量明显减少。 结论N-乙酰半胱氨酸通过增加心肌组织还原型谷胱甘肽含量,改善心肌组织氧化还原状态,从而减缓高血压大鼠心脏的进一步重构。
