针刺曲池、外关穴对脑卒中偏瘫上肢H反射、F波影响的临床研究

摘要
目的:研究针刺患侧曲池、外关穴对脑卒中存在上肢运动功能障碍的患者(Brunnstrom分期Ⅲ-Ⅳ期且改良Ashworth 1-2级)偏瘫上肢运动功能的影响,并使用电生理指标H反射、F波探讨其在脊髓环路可能的机制。方法:将符合纳入标准的48例患者随机分为针刺组和伪针刺组,每组24例,针刺组给予患侧曲池、外关穴针刺;伪针刺组给予患侧曲池、外关穴伪针刺;分别在针刺前5分钟、拔针后对两组患者进行评估;使用Hmax/Mmax及Fmax/Mmax评价患者的脊髓前角运动神经元的兴奋性;采用wolf运动功能评价量表(WMFT)来评价患者上肢运动功能;采用改良Ashworth量表(MAS)评价患者的肌张力。结果:1.与伪针刺组比较,针刺组拔针后,Hmax/Mmax、Fmax/Mmax及WMFT评分方面,差异有统计学意义(P<0.05),在MAS评分方面,差异无统计学意义(P>0.05);2.针刺组在拔针后,与针刺前相比,Hmax/Mmax、Fmax/Mmax及WMFT评分方面,差异有统计学意义(P<0.05),在MAS评分方面,差异无统计学意义(P>0.05);3.伪针刺组在拔针后,与针刺前相比,Hmax/Mmax、Fmax/Mmax、WMFT、MAS评分方面,差异无统计学意义(P>0.05)。结论:1.偏瘫上肢针刺曲池、外关穴后可以通过wolf运动功能测试量表发现患者上肢运动功能改善;2.偏瘫上肢针刺曲池、外关穴后患者上肢功能的改善与改良Ashworth的降低没有直接关系;3.针刺患侧曲池、外关穴对上肢功能的改善,可能部分来源于对脊髓环路的调节作用,表现为Hmax/Mmax、Fmax/Mmax降低。
