奥卡西平与卡马西平单药治疗部分性癫痫的对照研究

摘要
目的:以卡马西平为对照,前瞻性地研究奥卡西平单药治疗部分性癫痫的有效性和耐受性。 方法:对82名部分性癫痫患者随机给予奥卡西平或卡马西平单药治疗,其中奥卡西平组40名,卡马西平组42名,前瞻性地对这两组患者进行1年的临床对比观察。 结果:奥卡西平组服药后治疗失败率为15%,卡马西平组21.4%,两组无显著性差异。奥卡西平组与卡马西平组治疗后3个月发作完全控制(无发作),显效(频率减少>50%),无效(频率减少<50%)以及恶化(频率增加)患者的百分比均无显著性差异。治疗后12个月两组发作完全控制(无发作),显效(频率减少)50%),无效(频率减少<50%)以及恶化(频率增加)患者的百分比亦无显著性差异。奥卡西平组治疗后达半年无发作的患者占72.5%(29例),卡马西平组仅占57.1%(24例),两组百分比有一定差异,但不具有显著性意义。两组达到临床半年无发作所需的时间以及首次发作时间均无显著性差异。奥卡西平组副作用发生率25.0%,卡马西平组副作用发生率50.0%,具有显著性差异。治疗后3个月两组副作用发生率并无显著性差异,但治疗后6个月奥卡西平组副作用发生率低于卡马西平组,具有显著性差异。卡马西平组中常见的副作用有脱发,皮疹,嗜睡以及记忆障碍;奥卡西平组中常见的副作用有脱发,皮疹,体重增加,嗜睡以及眩晕。 结论:奥卡西平单药治疗部分性癫痫的有效性与卡马西平相当。奥卡西平的副作用发生率远低于卡马西平,且副作用最易发生于用药初期。奥卡西平与卡马西平均有脱发,皮疹以及嗜睡这些副作用发生,而体重增加和眩晕可能更易发生于奥卡西平,记忆障碍可能更易发生于卡马西平。
