外周T细胞淋巴瘤患者外周血中MDSCs比例及其临床意义的研究

摘要
目的:探索外周T细胞淋巴瘤(PTCL)患者外周血中髓源性抑制细胞(MDSCs)比例及血清Arg-1、iNOS、VEGF的表达水平与预后的关系。方法:⑴取25例PTCL患者,15例同期健康体检者(正常对照组)外周血,采用流式细胞仪检测其外周血MDSCs/单核细胞比例,分析MDSCs比例在具有不同临床特征的PTCL患者中和健康人中的差别。⑵应用酶联免疫吸附试验(ELISA)检测25例PTCL患者及15例健康对照者血清中Arg-1、iNOS、VEGF的表达水平。结果:(1)外周血中MDSCs比例与血清Arg-1、iNOS、VEGF浓度在PTCL患者中的表达呈正相关。(2)①MDSCs在PTCL患者外周血单核细胞中所占的比例显著高于正常对照组[(4.59±1.41)%vs(1.36±0.59)%,P<0.05]。②PTCL患者化疗后外周血MDSCs的比例较化疗前明显降低(1.29±2.95%,P<0.05)。③结外侵犯部位、骨髓侵犯、临床分期、IPI评分不同的PTCL患者外周血MDSCs的比例差异有统计学意义(P<0.05),即结外侵犯部位≥2个、有骨髓侵犯、临床分期Ⅲ-Ⅳ期、IPI评分为高中危-高危者,其MDSCs比例显著增高;而MDSCs比例与年龄大小、LDH水平高低及包块大小无关(P>0.05)。(3)①PTCL患者血清Arg-1、iNOS、VEGF表达水平显著高于健康对照者(17.09±2.56 ng/ml vs 10.93±1.08 ng/ml,P=0.000;21.90±3.65 Iu/ml vs 16.39±6.08 Iu/ml,P=0.010;37.46±38.73 pg/ml vs15.82±8.37 pg/ml,P=0.012)。②化疗后Arg-1、iNOS、VEGF表达水平显著下降。③结外侵犯部位、骨髓侵犯、临床分期、IPI评分不同的PTCL患者血清中Arg-1、iNOS、VEGF浓度差异有统计学意义(P<0.05),即结外侵犯部位≥2个、有骨髓侵犯、有巨大包块(直径≥10cm)、临床分期为Ⅲ-Ⅳ期、IPI评分为高中危-高危者,其血清Arg-1、iNOS、VEGF浓度显著增高。结论:⑴PTCL患者外周血中MDSCs比例与血清Arg-1、iNOS、VEGF浓度呈正相关。⑵外周血中MDSCs所占的比例及血清Arg-1、iNOS、VEGF表达水平可能是PTCL患者早期诊断、预测化疗疗效、判断预后的指标。
