天灸疗法防治儿童哮喘的疗效观察及其对TGF-β1和Smad3表达的影响

摘要
目的:观察天灸疗法防治儿童支气管哮喘的临床疗效,并在临床研究的基础上研究天灸疗法对TGF-β1和Smad3表达的影响,探究其防治儿童支气管哮喘的作用机制。方法:2013年07月至2015年08月期间于我院中医科诊断为哮喘的患儿80例,均符合哮喘缓解期中度持续和气虚质的诊断标准,分为西药组和中药组,各40例,中药组仅使用天灸疗法。其余40例分入西药组,患儿规律使用布地奈德吸入治疗。对照组随机选取与患儿同地区且与无血缘关系、无其他疾病的健康儿童20例,源于我院外科。临床研究中将中药组和西药组患儿治疗前后的症状、证候和临床疗效等方面进行对比。实验研究中用流式细胞术对两组治疗前后、对照组的TGF-β1和Smad3的表达量进行对比,并采用SPSS17.0进行统计学分析。结果:1.临床研究部分:1中药组和西药组在年龄、性别、病程等方面,差别无统计学意义(P﹥0.05),具有可比性。2两组在鼻塞喷嚏、咳嗽、喘息气促等作用相似(P﹥0.05);但中药组在改善气虚质的自汗、神疲和乏力方面比西药组作用更加明显(P﹤0.05)。3两组治疗前后总患病率均有所减少(P﹤0.01)。2.实验研究部分:1中药组和西药组在治疗前由流式细胞术所检测的TGF-β1和Smad3的表达量均无显著差异(P﹥0.05)。2两组治疗前TGF-β1和Smad3的表达量分别与正常组相比均有显著统计学意义(P﹤0.01)。3两组治疗后TGF-β1和Smad3的表达量与治疗前相比均有所减少(P﹤0.01)。4两组治疗后TGF-β1的表达量与正常组相比无显著差异(P﹥0.05)。5两组治疗后Smad3的表达量与正常组相比均有差异(P﹤0.01)。6而中药组较西药组在降低Smad3表达量方面稍好(P﹤0.01)。结论:天灸疗法和布地奈德吸入疗法对于鼻塞喷嚏、咳嗽和喘息气促这些主症方面作用相似,但是对于气虚质的自汗、神疲和乏力天灸疗法作用明显。在实验研究中天灸疗法和布地奈德吸入疗法均可以将TGF-β1的表达量调控至接近正常,但是天灸疗法在调控Smad3表达量方面更加明显。
