CaN-NFAT信号通路在大鼠经皮血管成形术后再狭窄中的作用

摘要
研究经皮冠状动脉介入治疗(PCI)术后再狭窄的发病机制以及寻找有效的控制措施是当前的热点。支架置入可以将再狭窄发生率从球囊血管成形术后30%-50%降低到10%-20%,随着抗细胞增殖药物洗脱支架(DES)的发展,再狭窄的预防取得了突破性进展。最近的资料证明DES可将再狭窄发生率降低到0-8%。但支架术必然会引起内膜损伤导致炎症反应。血管损伤时,多种炎症因子介导血管平滑肌细胞(Vascular smooth muscle cell,VSMC)从血管中层向内膜迁移,并在内膜层增殖,导致管腔狭窄。而其中许多与再狭窄相关的炎症因子都是Ca2+/CaM-CaN-NFAT信号通路介导起作用的。根据上述推测,Ca2+/CaM-CaN-NFAT信号通路在再狭窄形成的两个关键环节(炎症、VSMC迁移及增殖)中起作用。 研究目的: 研究CaN-NFAT信号通路在经皮血管成形术(PTA)术后再狭窄中的作用,以及CaN特异性抑制剂环孢素A(Cyclosporin-A,CSA)、葛根素对大鼠PTA术后内膜增殖的影响,为防治血管再狭窄提供新的理论依据。 方法: 雄性SD大鼠48只,随机分为假手术组(n=12)、球囊组(n=12)、环孢素组(n=12)、葛根素组(n=12)。环孢素组、葛根素组大鼠行PTA术前3天开始分别每天灌喂CSA(12.5 mg/kg.day)、葛根素(50 mg/kg.day),假手术组、球囊组大鼠灌喂等容积的自来水。PTA术后30天取材,血管组织作HE染色、免疫组化检测钙调神经磷酸酶(Calcineurin,CaN)在血管壁中的表达,光学显微镜观察病理学改变;Rea1 time PCR技术检测血管壁组织中CaN mRNA、NFATc3 mRNA的表达变化;ELISA法测定血浆MCP-1水平。 结果: PTA术后血管壁出现新生内膜,形成的新生内膜厚度不均。环孢素组与球囊组相比较内膜增生、内膜/中膜厚度明显减轻(P<0.05)。球囊组大鼠PTA术后,损伤血管壁组织CaN mRNA、NFATc3 mRNA表达明显升高,与环孢素组、假手术组相比差异有统计学意义(P<0.05)。免疫组化检测球囊组血管壁上CaN表达明显增强,与环孢素组、假手术组相比差异有统计学意义(P<0.05);血浆炎症因子MCP-1水平球囊组与环孢素组、假手术组相比差异有统计学意义(P<0.05)。葛根素组与球囊组相比,新生内膜、CaN mRNA、NFATc3 mRNA、CaN蛋白表达、血浆MCP-1均无统计学意义(P>0.05)。 结论: 1、Ca2+/CaM-CaN-NFAT信号通路在再狭窄形成的VSMC迁移及增殖中起作用。 2、Ca2+/CaM-CaN-NFAT信号通路在再狭窄形成的炎症过程中起作用。 3、CSA通过抑制Ca2+/CaM-CaN-NFAT信号通路减轻PTA术后内膜增殖。而且能降低PTA术后大鼠血浆的炎症因子MCP-1水平。
