缺血性脑卒中后免疫反应及免疫抑制相关研究进展

摘要：

缺血性脑卒中是中枢神经系统损伤最常见的原因之一。脑缺血后的脑损伤涉及复杂的病理生理机制,炎症免疫过程在其中起着基础性的调节作用。血流骤然中断引起血管内皮细胞坏死,早期诱发外周白细胞在损伤脑区募集,晚期淋巴细胞也迁移至梗死部位,产生急性炎症反应;同时,由于血脑屏障被破坏、脑组织肿胀以及坏死神经元触发小胶质细胞等的激活释放炎症介质,共同促使炎症瀑布形成,最终导致脑组织发生严重病变而产生神经功能缺陷。但过度的炎症反应会诱发免疫抑制,虽然能够缓解炎症带来的损伤,但可加重感染的风险。因此,深入研究缺血性脑卒中后免疫反应和免疫抑制的发生机制、探索相关治疗策略,对临床工作具有重要的指导意义。
