MoCA与MMSE在急性缺血性卒中后认知障碍评估中的应用

摘要
目的比较蒙特利尔认知评估量表(MoCA)和简易智能精神状态量表(MMSE)在急性缺血性卒中后认知障碍评估中的应用价值。方法 201例急性缺血性卒中患者, 采用MoCA和MMSE在卒中发病2~3周进行神经心理学评估, 以MoCA<23分和MMSE<26分作为是否存在认知障碍的分界值, 分析两种测评工具识别卒中后认知障碍的能力及二者的相关性。结果 201例急性缺血性卒中患者, MoCA和MMSE评分分别为(20.5±4.3)分和(25.4±3.5)分。MoCA和MMSE对急性卒中后认知障碍的检出率分别为57.2%和43.3%, 其中47/201例(23.4%)MMSE正常的患者MoCA显示存在认知障碍。MoCA与MMSE评分显著相关(Pearson相关系数r=0.833, P<0.01);一致性检验显示Kappa值为0.532, P<0.01。结论 MoCA对卒中后2~3周认知障碍的检出率高于MMSE, 但二者具有较高的一致性。
