替格瑞洛在急性冠脉综合征治疗中的应用观察

摘要：

目的观察急性冠脉综合征患者应用替格瑞洛抗血小板治疗的近期疗效和安全性。方法 300例急性冠脉综合征患者,随机分为替格瑞洛组和氯吡咯雷组,各150例。在常规治疗基础上,替格瑞洛组首次负荷剂量180 mg,然后以90 mg、2次/d维持剂量1 a,PCI前可再应用90 mg;氯吡格雷组中已服用者不给予负荷剂量,初次服用者给予300 mg负荷剂量,然后以75 mg、1次/d维持剂量1 a,PCI前可再应用300 mg。对两组治疗7 d、1个月时心因性死亡、顽固性心绞痛、心肌梗死发生例数及治疗7 d出血情况进行比较。结果替格瑞洛组治疗7 d时发生主要不良心血管事件和出血事件与氯吡格雷组比较差异无统计学意义。治疗1个月时,替格瑞洛组发生心因性死亡5例、心肌梗死5例、顽固性心绞痛11例,氯吡格雷组分别为12、14、22例,两组比较,P均<0.05。结论替格瑞洛治疗急性冠脉综合征患者的近期疗效优于氯吡咯雷,而且不增加出血风险。
