高压氧对脑梗死后出血转化的治疗作用及其机制研究

摘要
目的探讨高压氧对脑梗死后出血转化的治疗作用及其可能机制。方法将72只SD大鼠按随机数字表法分为假手术组、模型组和高压氧组, 每组24只。模型组和高压氧组采用线栓法致大脑中动脉缺血再灌注损伤, 并在再灌注前给予腹腔注射高糖制备成脑梗死后出血转化模型, 其中高压氧组于术后3 h给予高压氧治疗。术后24 h, 采用神经功能评定量表对各组大鼠进行神经功能评分, 采用2, 3, 5-氯化三苯基四氮唑(TTC)染色检测梗死灶体积百分比, 采用分光光度法检测血红蛋白含量以评估出血转化量, 采用Western blotting检测核转录因子-κB(NF-κB)蛋白的表达水平, 采用酶联免疫吸附法(ELISA)检测白介素(IL)-1β、肿瘤坏死因子-α(TNF-α)浓度。结果与模型组比较, 高压氧组的神经功能评分明显增高[(11.50±1.64)分vs. (16.04±2.09)分], 梗死灶体积百分比明显降低(33.7%±3.8%vs. 21.6%±4.1%), 血红蛋白含量明显降低[(11.94±2.26) μLvs. (8.21±1.96) μL], NF-κB蛋白表达水平明显降低(3.05±0.19vs. 2.26±0.25), IL-1β浓度明显降低[(49.78±5.65) pg/mLvs. (31.88±2.96) pg/mL], TNF-α浓度明显降低[(31.11±4.55) pg/mLvs. (19.39±3.19) pg/mL], 差异均有统计学意义(P<0.05)。结论高压氧对脑梗死后出血转化有治疗作用, 可减少出血量及梗死灶体积, 其机制可能与高压氧抑制NF-κB、IL-1β、TNF-α等炎性因子的表达, 减轻炎症反应有关。
