结直肠癌术后切口感染与机体营养状态、免疫功能及炎性因子的关系

摘要
目的:研究结直肠癌术后切口感染与机体营养状态、免疫功能及炎性因子的关系。方法:选择在我院接受结直肠癌根治术的146例患者,根据术后切口感染情况分别感染组和非感染组,手术前1天和手术后当天、手术后3天时采集血清并测定营养指标、炎性因子,采集外周血并测定免疫细胞含量、红细胞免疫分子含量。结果:术后当天,两组血清中Hb、TP、Alb、PA、Tf的含量与术前1天比较无显著性差异,外周血中CD3+CD4+、CD3+CD8+、CD16+CD56+、CD19+、CR1、CR3、CD58、CD59的含量均显著低于术前1天且术前1天时及术后当天,感染组血清中Hb、TP、Alb、PA、Tf的含量以及外周血中CD3+CD4+、CD3+CD8+、CD16+CD56+、CD19+、CR1、CR3、CD58、CD59的含量均显著低于非感染组;术后3天时,感染组血清中TNF-α、PCT、IL-1β、MCP-1、hs-CRP的含量均显著高于非感染组且与血清中Hb、TP、Alb、PA、Tf的含量以及外周血中CD3+CD4+、CD3+CD8+、CD16+CD56+、CD19+、CR1、CR3、CD58、CD59的含量呈负相关。结论:围手术期营养状况低下、免疫功能抑制会增加切口感染的发生风险且与炎症反应程度密切相关。
