集束化镇静镇痛干预策略的临床应用研究

摘要：

[目的]探讨集束化镇静镇痛干预策略(ABCDE Bundle)对机械通气(mechanical ventilation,MV)病人的临床应用研究。[方法]按入科顺序编号,将2015年5月—2016年10月重症监护室(ICU)收治的143例机械通气病人分为两组,应用横断面整体前后对照研究方法,常规组(70例)采用常规镇痛镇静护理。Bundle组(73例)应用ABCDE Bundle。监测两种干预方法:常规组和bundle组在实施后1d(T1)、3d(T 3)、5d(T5)、7d(T7)脱机人数的变化,记录相关预后指标。[结果]Bundle组脱机人数高于常规组(P<0.05);Bundle组预后优于常规组,差异有统计学意义(P<0.05)。[结论]ABCDE Bundle可减少机械通气病人的气管插管时间,缩短ICU住院日,降低28d死亡率和谵妄发生率,改善预后。
