重复经颅磁刺激和计算机辅助认知训练改善轻度认知功能障碍患者认知的临床研究

摘要
目的探讨重复经颅磁刺激(rTMS)和计算机辅助认知训练对轻度认知功能障碍(MCI)的干预作用。方法选择MCI患者72例,随机分为常规药物组、rTMS组、认知训练组、联合组,每组18例。另选健康体检者18例为对照组。所有受试观察前后均进行EEG、事件相关电位(P300)及临床记忆量表测试,并进行组间比较。结果常规药物组、rTMS组、认知训练组、联合组治疗后记忆商较治疗前显著增高(P<0.01),P300潜伏期较治疗前显著缩短[(301.5±31.5)ms vs(340.7±46.0)ms,(296.7±36.6)ms vs(341.6±61.2)ms,(302.6±30.1)ms vs(355.8±30.1)ms,(283.9±24.4)ms vs(341.7±41.9)ms,P<0.01];联合组P300潜伏期变化率、记忆商变化率明显高于常规药物组、rTMS组、认知训练组(P<0.05,P<0.01);认知训练组治疗后FP1、FP2、F3、F4、F7、F8、T3、T4、T5、T6导联,rTMS组FP1、FP2、F3、F4、F7、F8、T4、T6导联,联合组FP1、FP2、F3、F4、F7、F8、T3、T4、T5、T6导联脑电Lemple-Ziv复杂度较治疗前明显增高(P<0.05),而常规药物组仅FP2、F4、F7导联脑电Lemple-Ziv复杂度较治疗前明显增高(P<0.05)。结论通过rTMS、计算机辅助认知训练,可一定程度上改善MCI患者的认知功能,且联合治疗更为有效。同时因正常人群存在增龄性认知功能下降,也应适当加以干预。
