抑郁症患者血清脑源性神经营养因子水平及相关分析

摘要
目的探讨重性抑郁障碍与心境恶劣障碍患者血清脑源性神经营养因子(BDNF)水平,以及血清BDNF水平与抑郁症精神病理症状之间的关系。方法共有24例心境恶劣障碍患者,36例重性抑郁障碍(MDD)患者及27例健康志愿者参加此项研究,用汉米顿抑郁量表(HAMD)24评定抑郁症状严重程度,用酶联免疫吸附测定法测定血清BDNF含量。结果MDD组的血清BDNF水平明显低于心境恶劣障碍(DD)组及正常对照组(NC组),(12.45±1.20)μg/L vs(16.2±0.9)μg/L vs(17.21±1.31)μg/L,差异具有统计学意义(F=7.352,P=0.003),而后两者差异无统计学意义。抑郁组患者血清BDNF水平与病程负相关(r=-0.456,P=0.002),与HAMD24总分值负相关(r=-0.828,P=0.000)。进一步分析发现,血清BDNF水平与认知障碍因子和自杀因子(r=0.804,P=0.01,r=-0.807,P=0.001)负相关。结论MDD与DD存在异质性,血清BDNF水平与抑郁症自杀高度负相关,血清BDNF可考虑作为判断抑郁患者自杀的生物学指标之一。
