体外冲击波疗法在烧伤创面修复和烧伤后瘢痕治疗中的应用

摘要
背景:体外冲击波疗法作为烧伤治疗的新技术已开展16年,在促进烧伤创面愈合及改善烧伤后瘢痕挛缩、瘙痒疼痛及皮肤红斑方面取得了显著疗效,目前尚无规范性治疗指南,其分子机制、作用途径及治疗参数等值得进行更广泛深入的探索研究。目的:综述体外冲击波疗法在烧伤创面修复和烧伤后瘢痕治疗中的应用及研究进展。方法:在计算机中以"extracorporeal shock wave therapy,shock wave,burns,burn scar,scar"为英文检索词,以"体外冲击波疗法、冲击波、烧伤、瘢痕"为中文检索词,分别检索PubMed、Web of Science、中国知网、维普和万方数据库,检索时间截至2021年2月,得到关于体外冲击波疗法与烧伤、瘢痕研究的相关文献132篇,经纳入和排除标准筛选后,最终对59篇文献进行分析、归纳和综述。结果与结论:(1)体外冲击波疗法通过细胞机械转导、炎症反应调控、促血管生成改善循环及加速细胞增殖上皮化而促进烧伤创面的愈合;(2)体外冲击波疗法通过机械作用软化瘢痕,对瘢痕组织产生分子生物学调控,改善瘢痕瘙痒疼痛症状和瘢痕处皮肤外观及功能;(3)体外冲击波对烧伤的治疗作用体现为物理和生物学的双重影响,细胞机械转导途径在冲击波调控烧伤创面愈合以及增生性瘢痕进程两方面均发挥重要作用,冲击波的传导最终可引起蛋白、基因表达的变化并调控细胞生化反应。
